Introduction Thank you for choosing the Salumatics Coding Viewer (SCViSiON) System. This user’s guide is designed to provide an overview of this application. NOTE: The SCViSiON System was designed to provide the added security benefit of prohibiting users to update or change the information in the patient charts.

Security In addition to the standard PC and network security set up in your facility, the SCViSiON application provides a separate user login and password for access into the application. Users will have access to all functions such as Search and View. The Administrative User will have the additional functions of Audit Review which will allow the Administrator the ability to review charts accessed by function and by user and the ability to set up user profiles. NOTE: If a system is idle for more than five minutes the user will automatically be logged off. The User will be required to log back into the system for access to health records. This is a security feature to protect unauthorized access into patient records.

Upon opening the SCViSiON application, the Login Dialog Box Appears Getting Started Upon opening the SCViSiON application, the Login Dialog Box Appears Format for login is: first initial followed by then last name. NOTE: the login and password is case sensitive. LGOODWIN PASSWORD Click the OK button to continue A default password is provided for the first time the user logs in. The user can change their password by clicking on change password.

Search: Chart Selector 30 Method #1 – Search by MRN: Type in a Medical Records Number (MRN) and then click on the Search button Method #2 – Search by Chart List: The user can Click on the Search button to view the first 100 charts in the database. This option is limited to the first 100 charts.

Search Select the required visit by double clicking on the row with the corresponding visit NOTE: To search for additional charts, the user can replace the MRN number previously selected with the new chart number required.

Sort: Chart Selector A single click on the column heading will sort the information in ascending or descending order. Once the column is sorted, double click on the row to view the chart.

Displays key patient information View: By Document Type Displays key patient information Document List: Display the documents within the chart by document type. Users can click on the document type required and then the page Users have the option of viewing the chart in chart “Assembly Order” in addition to document type

Users can utilize the zoom icons located in the tool bar. View: Zoom In/Out There are a few options to zoom in and out on an image /document as required. By right clicking anywhere on the image, the zoom options will appear (such as increase by a percentage, Best Fit, Fit to Width, and Fit to Height) OR Users can utilize the zoom icons located in the tool bar.

First Image in the chart View: Multiple Pages Another option for Users to scroll through the chart is through the use of the arrow icons First Image in the chart Last Image in the chart Previous Image Next Image

View: Landscape For documents that require rotation for viewing, such as landscape documents (ECGs) or flow sheets, users can utilize the direction arrow icons. Rotate Left Rotate 180° Rotate Right

View: Back of Image To view back of image, click on “Back of Image”. To view the front of the Image again, click on “Front of image”

Viewer Input Screens Once the chart has been selected, there are four input screens in the Virtual Coding Application. The screens are designed for students to enter basic demographic, diagnostic, intervention and narrative information as they code a client/patient record. Complete each field as required. Instructors will be reviewing information entered.

Input Screen To begin entering data into the input screens, click on the “Input” icon

Chart Coding Form Screen Demographic Screen Upon selection of the Input Icon, the user will see this screen. There are four data entry screen options. Diagnosis Screen Intervention Screen Narrative Screen Summarizes the chart information, pulled from the database Summarizes the coder/user information, pulled from the user login/profile

Demographic Screen Users can now begin to enter data for each client record Inpatient 2003-11-25 2003-11-26 Dates may be entered in any format. The system default is YYYYMMDD. Urgent/Emergent 1978-06-22 Once information is completed, if user is logged in as a non-graded student, (they will be able to view the answer key) click on Female Discharged Home If user is logged in as a graded student (they will not able to view the answer key), and user is ready to submit answers, click on 53 50 The Administrator sets up the user profiles status (ie. Non-graded or graded student) No

Demographic Answers As a non-graded user, users can compare their answers to the formal answer. Click on the Close icon to continue. Users will be returned to the Coding Chart Screen Inpatient 2003-11-25 2003-11-26 Urgent/Emergent 1978-06-22 Female Discharged Home 53 50 No

Chart Coding Form Screen After closing the Demographic application, the user is defaulted back to Chart Coding Form Screen for selection of the next required input screen. NOTE: Users have the option of having both the input screen and the “chart” open at the same time. Closes the Window Diagnosis Screen Narrative Screen Intervention Screen Demographic Screen

Diagnosis Screen The Diagnosis Screen has similar functionality as the demographic screen. A maximum of 25 diagnoses may be captured per client record. Users are to use the “Tab” key to move between diagnosis code fields When all information is added, click on the view answers icon or submit answer icon (as appropriate) Non-graded, view answers Submit for grading Once completed, user can click on the Close icon to return to the Chart Coding Form Screen

Intervention Screen Five interventions may be captured per episode. There are a maximum of three episodes per client record. If there are greater than five interventions per episode, users are required to see their administrator/instructor. Tab between intervention code fields to add data. When all information is added, click on the view answers button or submit answer button (as appropriate) Non-graded, view answers Submit for grading User can click on the Close icon to return to the Chart Coding Form Screen

NOTE: Instructors are able to report on this field. Narrative Screen This narrative screen is a free text area for students to add detailed information regarding the logic behind code selection or other pertinent notes. Chart Coding Form Screen Upon completion of data entry in all of the applications (Demographic, Diagnosis, Intervention and narrative), the user has just completed coding the record User can click on the Close icon to complete the process of coding the record. Closing The Client Record After closing the Chart Coding Form application, to complete the process, the client record must now be closed before another record can be selected or before the Virtual Coding Viewer can be closed.

Once option is selected, click “OK” to continue. Chart Close Options Upon closing/completion of a chart, the following screen will appear. The user is to select the required option, such as “Complete”. Once option is selected, click “OK” to continue.

Closing the Viewer Upon closing/completion of a chart, the user can click on the Exit icon to exit from the Chart Selector screen.

Closing the Viewer cont… To close the viewer/application, users must click on the CLOSE icon in the top left hand corner. Do Not Use the “X” to close the application.
