Age of Death England, no LD = 78 men/83 women England, LD = 63 men/64 women Age of death in Derbyshire comes from 97 of 119 GPs year end March 2016 We had 72 deaths in 97 GP practices last year Derbyshire LD 55-64 men/ 65-74 women

Each Area to check Deaths of People with Learning Disability aged 4-74 Tell Bristol University about a death, by internet or phone Uni will tell Local Area Contact Louisa Whaite Louisa will tell Derbyshire Kate Majid We start April 2017 Death notification. Initial review. If further review is indicated, multiagency review and action plan. Action plans and recommendations agreed locally and taken forward into service improvements as necessary. National analysis of common themes and recommendations. National oversight of action plans and service improvements.

Initial Review Kate will tell a Reviewer Reviewer checks notes Interview with someone who knew the person well May be a multi agency review Will need to have a clear case note request process in place – contact details, expected timescales for return etc Local areas will need to consider how they will manage conflicts of interest – eg, if reviewer has had involvement with person – and how long ago Identify one set of case notes based on where person died Interview someone who knew the person well – family member, friend, support worker etc. Purpose is to develop the richness of detail about the events leading upto persons death and perceptions of treatment
