Ward 6 & Ward 12 Woodend Ward 6 Ward 12 16 beds in 2 Nightingale wards with the option of 2 single rooms 1 small therapy room 1 small dayroom Located on 1st floor Ward 12 22 beds 3 x 6 bed areas 4 single rooms 1 Therapy Gym 1 Therapy kitchen CHSS Office MDT Offices Self contained unit with access to garden & patio

ADMISSION CRITERIA WARD 6 Ward 12 Over 65 years of age Have experienced a recent stroke Would benefit from a period in a stroke-focussed unit for assessment and rehabilitation Be medically stable Ward 12 School leaving age to 65 years Have sitting balance or the potential for Have the ability to participate in a rehabilitation programme Be medically stable

Patient Profile Ward 6 Ward 12 9 Males 17 Males Age range 54 – 77 years Mean 68 years 7 Females Age range 64 – 86 years Mean 79 years Average length of stay = 172 days Ward 12 17 Males Age range 33 – 87 Mean 64 years 1 Female 65 years Average length of stay = 82 days

ADL Abilities Ward 6 Ward 12 0 requiring minimal or no assistance 4 requiring moderate assistance ie. Help with dressing/toileting/mobility (25%) 12 requiring all care (75%) Ward 12 6 patients requiring minimal or no assistance (33%) 5 requiring moderate assistance (28%) 7 requiring all care (39%)

Destination on Discharge Ward 6 6 patients will be going home, -most of other 10 patients to Nursing Home Ward 12 12 patients going home, remaining 6 to Nursing Home

Issues common to both areas - Shortage of therapy staff - Setting up home care often takes many weeks ….delaying discharge Patients coming sooner after stroke Move to combined unit on ground floor of Westview
