Engaging Patients and Consumers How can Connecting Communities teams engage consumers in the design, awareness of, and use of EHR and PHR systems? How should Connecting Communities teams evaluate and prioritize the implementation of patient-facing EHR or PHR application as they roll out their program? What lessons have been learned from early PHR projects that can help Connecting Communities teams plan for their own work? How much should Connecting Communities projects work together or conform to national standards vs. just solve local problems as local interests see fit? How can features of the EHR roll-out be designed to maximize benefit to and use by patients and families?

Engaging Patients and Consumers How can Connecting Communities teams engage consumers in the design, awareness of, and use of EHR and PHR systems? Design with the patient/consumer in mind—don’t make their use of the system an afterthought o       User-friendly look and navigation o       Avoid medical jargon aimed at professionals o       Make relevance obvious throughout design Case Example: MI Counselor Design and voice

The value of the PHR will be maximized when: Consumers can use it to enhance their continuity of care Clinicians use PHR as vehicle for prescribing clinical content specific to individual moments in care PHR integrated with other electronic applications Interoperability efforts allow for universal use of PHRs

Vision of the Future: Seamless care achieved by linking PHR to clinical content for: Every medical test Every prescription medication Preventive services Epidemic or disaster response Specific chronic conditions (CAD, asthma, etc.)
