Gen-H is Going Beyond Healthcare We know that clinical care contributes to only 20% of overall health status. That’s why we’re activating data driven solutions to address the social and economic factors that impact the health of our region. Accountable Health Communities: Bridging the clinic to community gap by addressing social determinants of health Build Place Based Health: Providing technical assistance and data to local leaders in priority neighborhoods to help improve health outcomes Comprehensive Primary Care +: Motivating, supporting and rewarding providers for delivering better care and producing better outcomes

Accountable Health Communities The Accountable Health Communities (AHC) Model will assess whether systematically identifying the health-related social needs of community-dwelling Medicare and Medicaid beneficiaries (CDB’s), including those who are dually eligible, and addressing their identified needs, impacts those CDBs’ total health care cost and their inpatient and outpatient health care utilization.

Build Place-Based Health Coordinate learning collaboratives for existing place-based and neighborhood initiatives. Engage residents, neighborhoods and employers in regional Step Up Challenge

Step Up Cincinnati Challenge Integrate physical activity into our daily lives with the Step Up Cincinnati walking challenge. Collaborating with go Vibrant to create a real time leaderboard for participants to track their steps. Organizations will be recognized and awarded for their participation.

The Health Plan UnitedHealthcare This is a snapshot of our region now. We went from supporting 75 primary care practices to 562 across the entire state and in 4 counties of northern Kentucky. The payers in blue continued their participation and support from CPC Classic into CPC+. 562 practices 2,500 providers 3M+ patients
