Cardio Transport Process Map 2 1 HBS MD enters HC order Cardiologist approves order Cardiology calls Nuc Med to notify of approval Nuc Med calls floor to request patient RN calls HEN to request transport RN Inputs HC transport request—notifying transport RN preps patient for transport RN prints transport report for HEN HEN and transport arrive at patient room Transport, HEN, and patient go to Nuc Med Patient arrives at Nuc Med imaging room Transport helps move patient then leaves; HEN stays Patient gets resting image Patient and HEN go to cardiology 1. Patient gets stress test 2. Patient and HEN go back to Nuc Med to reimage 3. Patient goes back to cardiology and finishes stress images Nuc Med tech puts HC request for transport Transport goes to Nuc Med Transport, HEN, and patient leave Nuc Med Patient taken back to room Cardio Transport Process Map 2 1 Voice of the Patient Process Pain Point No or incorrect order in HC HEN not always available, ANM may need to find replacement Replacement may be break nurse which can lead to penalties for missed breaks Points along the process where time stamps are collected Carpet in hall slows down transport= increased risk of workplace injury & sometimes requiring a second transporter Nuc Med not always aware of delays= difficult to plan for patient with full clinic schedule. Monitors not always available, and far away to grab Phone interruptions Rounding and medication interruptions Patient frustration--“I will be so happy when this over” Patient concern—”Process doesn’t look coordinated”, lower confidence in care Patient anxiety—”What’s going on?” Patient not always ready when transport arrives Difficult to coordinate HEN and transport Sometimes cardiology cancels the test, but they do not notify transport team Sometimes transport info in HC is missing or incorrect Patient embarrassed—”I can walk. I wish I had my pants, underwear, bra on. Walking around in this open gown is very uncomfortable” 3 Narrow doorway slow down transport and increase risk of injury Nuc Med unable to identify isolation patients to ensure precautions taken 4 Transport info in HC can be missing or incorrect HEN has to wait for transport=$$ If no transport available, HEN takes patient back to room 5 Patient frustration--“I feel uneasy and confused being left in hallway, what’s taking so long?”
