Earning Points Basic Requirements (Complete One per Quarter) Physical Fitness (Earn 100 Points per Quarter) Other Activities (Earn 100 Points per Quarter) Visit PCP or complete a wellness screening Walk/run (3 points/day for 6,000+ steps) Get a flu shot (50 points) Complete a Health Risk Assessment Cycle (4 points/day for 3+ miles) Complete an e-training module (50 points) Visit the dentist Fitness facility for 30 min minimum (4 points/visit) Participate in a wellness challenge (50 points) Complete e-training module on Financial Wellness Volunteer (50 points) Complete CPR/defibrillator/ First Aid training (50/points) Donate blood (50 points) Participate in Real Appeal (100 points)

The Reward Qualifying employees receive $120 off health plan premiums every quarter they achieve 200 total points
