Saturday May 21, 2016 Join DDC on a journey back to the mid 1960’s where a pop group from Liverpool took the American Billboard Charts by storm! Practically unheard of prior their American debut, their quick rise to fame stunned even their biggest fans! This year, your dancer will perform to some of the most famous and widely covered music of all time! Dancers will perform in at least 2 dances on stage. Recital participation is entirely optional, however, it is highly encouraged as it gives your dancer an important sense of accomplishment after a year of dance! WE’LL SEE YOU AT THE SHOW! Recital Details: Recital Date: Saturday May 21, 2016* Recital Venue: Cherokee Arts Center Recital Cost: $100 *your dancer will be required to attend one mandatory dress rehearsal the week of the show. About 98% of our dancers participate in the show and costume availability greatly decreases as recital date approaches. Reserve your child’s place in this year’s show today! Dreamweaver Dance Company Recital Order Form/Release To Enroll in Recital: Fill out and sign the form on this sheet. Submit to DDC via email or mail along with your recital payment. PLEASE DO NOT LEAVE THIS FORM AT YOUR CHILD’S SCHOOL! PLEASE EMAIL TO US OR MAIL TO: Dreamweaver Dance Company 136 Oak Grove Place Acworth, Ga 30102 3. If you would like to use a credit card to pay for recital, please visit our website and choose the appropriate PayPal option. To be sure your dancer will receive a costume for this year’s show, recital forms and payments MUST be received by DDC NO LATER than January 15, 2015. As recital season approaches, costume companies are inundated with orders and costume availability greatly decreases. DDC cannot guarantee a costume if your form is received after Jan. 15. Since costume companies do not refund for cancelled costume orders, DDC does not refund recital fees. RECITAL COST AFTER JAN. 15TH IS $115. *Account needs to be current in order to participate in recital. Please note that any payment made to DDC will be applied toward tuition FIRST! All past due accounts must be paid in full by May 5th in order to receive your dancer’s costume and be eligible to participate in the performance on the 21st. My child has my permission to participate in DDC’s 2015 recital. By signing this form, I am agreeing to pay my recital fee in full prior to January 15 and that if my recital fee is not received by DDC prior to the deadline, I may be in jeopardy of receiving my costume late. I understand that by my child’s participation in the show, my child may be photographed or filmed for promotional purposes. I understand that there is no refund for my recital fee and I have circled to my best knowledge, my dancer’s estimated size in May and that any alterations that might be needed will be my responsibility.
