Drivers Safety scorecard and accident Statistics Construction Equipment & Fleet Services For purposes of this policy, a “motor vehicle accident” is defined as the following: The collision of a company vehicle with another vehicle, stationary object, road debris, animal, or person, resulting in injuries, death and/or loss of property. Contact the police and emergency services if there are injuries to persons and/or damage to other vehicles or property. It is a violation of this policy to leave the scene of the accident without the permission of law enforcement. Globally it is a standard practice to contact your direct supervisor and the local or regional Environmental Safety, Health & Security Services (ESH&S) Manager. Collect Information ·         A police report is required for all accidents where there are injuries to persons and/or damage to other vehicles or property. The driver must obtain as much information as possible at the scene of the accident, including names of the persons involved, ownership of the other vehicle, names of insurance companies and the names of witnesses. ·         Take photographs of scene if it is safe to do so. Make No Statements ·         Do not make any statements (other than to police, company officials, LeasePlan, and B&V insurance representatives). Do not sign anything presented by other parties involved in the accident. ·         Drivers should not acknowledge any fault at the scene of the accident ·         Check on others involved, but do not discuss the incident or give your opinion about what happened until the police or company officials arrive on-scene. ·         A witness can talk to you, but do not volunteer information about the facts of the accident. Post-Accident Drug and Alcohol Screening Where applicable and not prohibited by law, all Black & Veatch professionals reporting a motor vehicle accident will be required to submit for a drug and alcohol test at a pre-designated testing facility. The Manager of Safety Support Services will schedule all drug and alcohol screenings and maintain all related records in a separate locked confidential file. This will ensure consistency in testing and confidentiality of results and will comply with federal regulations for recordkeeping. Accident Review Board members include: Fleet Manager Immediate Supervisor (as required by the ARB) Division Safety Manager(s) Operations Manager(s) Insurance Legal (as required by the ARB)
