Eligibility Criteria Individual is literally homeless as defined by HUD Individual is experiencing chronic homelessness or is at risk of chronic homelessness. At risk of chronic homelessness being defined as minimum of 5 months continuous homelessness Both chronic and at risk of chronic require a disabling condition.

Eligibility Criteria Disabling conditions can be self reported Disabling conditions include: physical health, mental health, A & D, other (if other, please specify) A bus pass will not be approved without a disabling condition.

Eligibility Criteria Individual must be on the By Name List An individual is placed on the By Name List by adding an Entry under the Enter Data As (EDA) mode of Nash TN CES (411) in HMIS. Be literally homeless Answered “yes” to the question of wanting to work towards housing.

Eligibility Criteria Individual must be willing to work with Housing Navigator Engage in housing search Use bus pass to engage in housing search, visit other service providers, support services etc. Have regular follow ups with navigator

Application Process Housing Navigator will submit form on behalf of client through the Weebly site. Information includes length of homelessness, disability type, contact info Submission includes signed HMIS release, signed Code of Conduct and photograph all uploaded into HMIS. Commission staff will verify all documents are uploaded before approving pass. All 3 documents must be included.

After approval Once approved: MHC notifies housing navigator of approval and prints bus pass MHC will bring the printed bus pass to an Individual, Veteran or Family Care Coordination meeting and give to the Housing Navigator or a representative from the navigator’s agency. If prior arrangements are made, navigators make pick up card from MHC staff.

Navigator commitment Check in with MHC through Care Coordination meetings to report progress towards housing and service engagement. Use HMIS case notes to document contact every two weeks Enter case notes under EDA mode Nash TN CES (411) Information in case notes Ex: - VISPDAT completed - Linkage shown to other mainstream services - housing plan update and main housing barriers

Deactivation/Reactivation Every 90 days MHC staff will check HMIS for case notes reporting contact. If a housing navigator loses contact with individual The card will be deactivated. If navigator regains contact, a new card can be applied for. If a housing navigator does not document contact MHC staff will notify housing navigator regarding potential deactivation. The housing navigator will have one business day to update HMIS to reflect contact.

Deactivation/Reactivation If a bus pass is deactivated due to violation of the terms of code of conduct, the navigator can advocate on behalf of client at care coordination meeting. MHC director must approve an appeal to MTA to lift conditions of the violation MTA has the final decision over deactivation due to code of conduct violation.

Lost Connector Card If a bus pass recipient loses a bus pass, it will be deactivated and a new bus pass will be made available (no questions asked). A housing navigator will need to fill out a new application each time a new bus pass is requested. Requests for re-activation after a second loss will require approval by MHC director.
