Serious Adult Review SSAB Commissioned a review following the suicide of an adult who was admitted to hospital following an overdose. He left the hospital alone although he was deemed to be ‘at high risk of suicide’ and in need of 1:1 supervision. The aim of the review is to learn the lessons from the ‘case’.

Serious Adult Review SSAB Commissioned a review following the suicide of an adult who was admitted to hospital following an overdose. He left the hospital alone although he was deemed to be ‘at high risk of suicide’ and in need of 1:1 supervision. The aim of the review is to learn the lessons from the ‘case’, and improve future practice.

Key lessons from the SAR Information Sharing, communication and terminology e.g. medically fit ? What we mean by ‘supervision, and 1:1 ? Management of people who self harm. Out of hours arrangements e.g. warrants. Missing person’s policy. Enhanced supervision policy. Escalation process e.g. staff cover. Discharge planning. Risk Assessments. Meeting mental health needs in an acute setting.

Domestic Abuse Updates Multi-agency training planned on October 28th Better engagement with perpetrators of Domestic Abuse. To include current research and practice developments, techniques to improve engagement and risk assessment. The plan is to develop a training resource for local agencies to use from 2017.
