Patient-centered care According to the patients. Groningen 7 November 2016

Whom do we want patient-centered care for? What is it according to the patients What are the benefits for the patients of patient-centered care? Where do we start with this? What is needed for this? Hurdles to overcome to perform patient-centered care What can be done?

For whom do we want patient-centered care? Chronic patients? Everyone?

What is it according to the patients Definitions The experience (to the extend of the informed, individual patient desired it) of transparency, individualization, recognition, respect dignity and choices in all matters without exceptions. Related to one's person, circumstances and relationships in healthcare. (Berwick, Donald July 2009) Patient-centred care is providing care that is respectful of and responsive to individual patients preferences, needs and ensuring that patients value guide all clinical decisions. (Institute of medicine US 2001)

Patient-Centered care is care provided by professionals and caregivers in which the patient stays in command and acts from the perspective that he is been given an understandable overview of options, combined with a benefit / risk analysis so he is able to make a well-informed choice.
