We are not asking anything about clients current premium, when they last claimed, how many claims they have had or when they had inpatient or diagnostic tests. Quotes must be produced between 1st November and 31st December. You can backdate quotes 30 days and future date 30 days. Must have 12 months cover with the last provider. Must be on a consumer policy currently. Add-ons can be added to the policy inc. travel Can pay from a company bank account – with company mandate form. No Months Free on inception. Switch Dec 1. In the last five years, has the person to be covered had ANY form of cancer that is not already excluded on their existing scheme? NOTE - If a customer has been signed off from the consultant but is having annual check ups, you can answer as “NO”, as long as the treatment was completed more than 5 years ago. NOTE - Basal cell carcinoma and Squamous carcinoma can be answered as “NO” if one episode and signed off from a consultant more than 1 year ago. 2. In the last five years, has the person to be covered had a heart or circulatory condition that is not already excluded on their existing scheme? For example coronary artery disease (angina & heart attack), stroke, abnormal heart rhythm, blood clot, aneurysm, heart valve disorders, high blood pressure/cholesterol, etc  NOTE - If a customer states that they have had uncontrolled blood pressure / cholesterol in the last 5 years, but this has been controlled for the last 2 years then you can answer “NO”. In the last two years has the person to be covered had any arthritic conditions, and / or back and neck conditions and / or spinal conditions (including sciatica), or joint replacements? NOTE - You only need to answer Q3 as “YES” if the customer has been diagnosed with a medical condition. If a customer mentions they get “aches and pains” but they are not being seen by a doctor, this can be answered as “NO”. 4. Does the person to be covered have ANY planned or pending investigations, treatment, surgery, for any condition or symptom? (This applies whether the treatment is planned privately or under the NHS) NOTE – don’t not need to declare yes for the following: Peace of mind checks that the customer is paying for themselves. Routine checks for chronic conditions if they are seeing their GP with no specialist involvement or further investigations required. For anything that is not covered under our general exclusions i.e. dentistry, sleep conditions etc. If treatment complete and there is a follow-up consultation but no investigations or further symptoms/ treatment.
