SPLINTING
MLREMS Collaborative Protocols https://www.mlrems.org/provider/protocols.aspx (Online PDF Version) App Let’s make sure everyone is on the same page for the Protocols
These are the protocols we practice in the field in this region- Monroe/Livingston Regional EMS Council
NYS Protocols are for EMT class- this is what is tested on the state exam, it is not region specific PLEASE STUDY THIS VERSION IF YOU ARE TAKING THE EMT CLASS NOW - Bruce has not handed them out, but read them every day when you get them
What is the Main Purpose of Splinting? Immobilize an injury and prevent further damage (while minimizing pain) MAKE THE PATIENT COMFORTABLE Also keep in mind that actual patients may not be able to move their injured extremity to a position that is convenient for splinting
Traction Splinting- KTD https://www.youtube.com/watch?v=QIxzroh2rhw Manual stabilization CMS Ankle strap Ischial strap Measure tent pole and fold into ischial strap Put on the knee strap Hook the ankle strap to the tent pole connecter Pull mechanical traction Attach thigh and shin strap Recheck CMS Transport on the backboard
Traction Splinting - Hare Picture- makeshift ankle strap with Cravats We only have the hare: skinny straps, do not put them directly on the break or on the knee: Order Manual stabilization CMS Measure traction splint on non-injured leg and adjust it Ankle Strap Manual Traction and switch to partner who was holding stabilization Slide the splint under the leg as far up to the edge of the buttocks as possible Ischial Strap Connect ankle strap to hook on traction splint Pull mechanical traction: pull until the tension is equal Straps on Check straps are tight Recheck CMS Transport on a backboard by securing splint and pt to backboard
Long Bone – Radius/Ulna Hand rest Can also be done with one board: similar to wrist splinting which you will see in a minute Pt. comfort
Long Bone – Tib/Fib Secure the foot with an ankle strap Trick with stepping on cravats to tighten them the most
Joint – Elbow Do not let anything touch the elbow directly Make sure to attach the cravat to the boards AND the pt’s arm
Joint- Knee Make sure to apply the cravats around the pt’s leg AND the boards for the 2 middle straps. Then put the other two either surrounding the leg and the board, or just the boards --> tighter this way
Wrist with SAM Splint Joint- Shoulder Shoulder should be raised you will most likely find them in this position SAM splints can be folded and molded to the position you need; we do have bigger SAM splints, but they are not always as secure as short boards- use caution. Tiny ones are good for wrists
