HEALTH COACHING WORKFLOW Will the patient benefit from health coaching? Is the patient open to health coaching? Physician and coach huddle to cover the physician’s goals for the patient and review the patient’s medications Health coach explains what coaching is to the patient Patient with diabetes meets with physician INITIAL COACHING SESSION Warm handoff Yes Coach uses the physician’s notes and after-visit summary to close the loop on that day’s care plan No Does patient agree to future session with health coach? Next appointment with physician is scheduled No No Yes FOLLOW-UP COACHING SESSION(S) Does the patient understand? Is the patient motivated? Coach uses ask-tell-ask and teach back to engage the patient in creating an action plan Coach schedules coaching session several weeks after the initial encounter Coach may do medication reconciliation and adherence counseling Yes Coach engages the patient in discussing a behavior action plan.
