FOG ACCIDENT at 5c – 15 - n6 stope on 11 January 2018 injured injured Mr Duane Cloete Mr Sello Elias Montsi DESCRIPTION OF ACCIDENT On the 11 January 2018, approximately 10:00 am, a fall of ground occurred between the face and the last row of support +-11m above the ASG. Mr Montsi(Winch Operator) was travelling towards the bottom of the panel when he was struck by a fall of ground. Mr Cloete (Miner)went to assist Mr Montsi and was struck by rocks from a second FOG from the same area.

Lessons learnt Entry Examination and safe declaration not conducted by crew Line Supervisors failed to identify the reasons that lead to the accident original waiting place was on 4 level Crew decided to move waiting place to 5 level Miner and Team Leader entered raise from 4 level Confusion of the supervisors

FOG ACCIDENT at l8A – 28 – n2 stope on 22 January 2018 injured injured injured MR. YANGA KHATSHWA MR. MPHO RAMABANTA MR. LENDELANE JAFETA DESCRIPTION OF ACCIDENT   On the 22 January 2018, approximately : 10:45am, while conducting sweeping operations against the top abutment a rock dislodged from the abutment sidewall (Scaling Rock) and struck the now injured persons.

Lessons learnt No entry examination done by crew Lack of involvement by supervisors New Team Leader(1st shift with this crew) Miner not involved as he was in the x/c arranging the tramming The plan was to do sweepings – crew went in to the back area and started with sweeps Team Leader in the ASG with other crew members
