Increased Access to Care Logic Model SHORT-TERM OUTCOMES INTERIM OUTCOMES LONG-TERM OUTCOMES INPUTS STRATEGIES Increased awareness of policies supporting delivery of quality services and access to affordable care for Kansans among government and system policymakers Effective policy passage, implementation, and enforcement/blockage of harmful policy Improved access to care: Increased number of Kansas adults with health insurance Support advocacy efforts to increase access to quality health care in Kansas Increased public pressure on policy makers at the state and local level. Build the capacity of providers to deliver more, high quality care Increased availability and quality of care: Improved health delivery systems, including availability of services Reduced health care costs Increased cultural competency of clinics and other service providers Foundation: Staff Reputation Partnerships with grantees and other stakeholders Experience Commitment Grant making Communications Policy Advocacy Other KHF focus areas Research and evaluation Improved capacity for advocacy (e.g. alliances and coalitions) among health advocates become stronger, more active and more inclusive of diverse constituencies Community norms support access to care and delivery of quality health care services Increase the capacity of clinics and other organizations to advocate for improved access Increase awareness and will around barriers to accessing care and potential solutions More services are available to meet the health needs of Kansans Impact: Improved health for all Kansans, because more Kansans can and do access quality health care coverage and services.
