AHA Rural Health Care Leadership Conference: HRET HIIN call for storyboards Storyboard Template| Due by 5:00 p.m. CT January 12, 2018 to hiin@aha.org

Instructions Please complete slides 3-10 of this template to describe your HIIN project. Use the questions on each slide as a guide to tell your improvement story; each question does not need to be answered in full detail. Your presentation must include exactly 8 slides. Storyboards should focus on success in a past or present quality improvement projects, emphasizing how they accelerated and amplified the work in the following topic areas: Readmissions Reduction Sepsis Transfer Health Equity Community Partnerships for Improved Health Outcomes Submit your storyboard to HIIN@aha.org by 5:00 p.m. CT on Friday, January 12. Awardees will be notified by Friday, January 19, 2018. If selected, HRET will finalize, print and ship your storyboard to Phoenix. Questions? Contact HIIN@aha.org.
