The Mission of Life Services To make a sustained impact in our member’s lives by effectively addressing the obstacles that impede progress in a member’s journey toward improved economic health, physical health & overall well-being. Improved Economic Health Improved Physical Health Self-sufficiency – maybe we say subsidy free? No one is really self sufficient but I think what we are striving for is economic independence? Improved Overall Well-Being

Greater Dayton Target Area CareSource has 102,922 members over the age of 18 in the Greater Dayton Target Area Goal for 2015 is to engage 1500 members

Unique Opportunity Life Services Health Care Including Mental & Behavioral Health Life Skills & Assessments Social & Community Needs Education & Training Job Connection & Retention Coaching & Mentoring CareSource Members are given access to holistic services CareSource Members are being assisted in all areas of their life with a coordinated holistic approach CareSource Life Services has tracked the number of organizations and programs that member utilize – on average members are engaged with 7-10 different organizations to help them survive month to month We are tracking the time it takes to see government & community resources replaced by income

Metrics Year to Date 630 266 121 65 50 92% Members have been hired. Members working with Advocates & Coaches to stabilize and find employment. 266 92% The retention rate for members who have been hired. 630 TO DATE: Members attended a Life Services event. 121 Members are enrolled in or have completed education training. 65 Members are pending employment. 50 Members have been hired.
