How do I refer a patient? REFERAL REQUIRED: We provide powerchart, email or telephone recs to referring provider We do not write scripts (few exceptions) Ad-Hoc I have discussed a palliative care referral with my patient If referred from inpatient, the outpatient provider managing their care is … xxx All referrals are screened prior to scheduling (no chronic pain, addiction management) Questions?
