Matching Data Fields Demographics must match on the Bright Futures application and student record in the Online Transcript Entry & Evaluation System: First Name Last Name Date of Birth Social Security Number (SSN), if provided on both the application and the student record

Check your date of BIRTH If we have your Social security, then it will be included on the sticker. If we do not have your social security number on file, bring a copy to Guidance.

Action Steps-Demographics If you notice an error, please raise your hand. If there isn’t any error, please sign your name on the paper. MUST TURN IN Your application must match what we have, so please don’t enter your Nickname on the application. Please include your entire last name (hyphen, or two last names, etc)
