Chalmette Refinery Good Catch 04-12-05

ICAR Date & Time Incident Started: 4/11/05 12:10:00 PM Incident Description: During inspection of K-2308 H2 make up compressor it was discovered that a bolt resting on the 4th stage suction piping had worn a hole in the pipe and caused the release of make up hydrogen. The compressor was taken off line the depressured.

The screw and nut mounted a temp gauge to a stainless steel plate.

Two mechanics from Dressor Rand happened to hear the leak when they were working the compressor and contacted our operators. The operators responded immediately and safely shut the compressor down
