Coal Mine Fatal Accident 2012 (No.2) OVERVIEW On Monday, June 25, 2012, a 33-year old outby foreman with 7 years of experience was fatally injured while installing a wooden timber next to a loose coal rib. He was positioned next to a loose coal pillar (rib), when a section dislodged and struck him resulting in fatal injuries. Mine management had increased the height of the mine entry to 8 feet and 6 inches by cutting additional mine roof. This action increased the rock thickness along the top of the coal ribs. The underlying coal seam had been mined, allowing additional pressure to be exerted on the standing coal pillar.

A co-worker was standing next to the belt structure and heard the dislodged material strike the mine floor. The mining height at the accident scene was 8 feet and 6 inches. Coal Mine Fatal Accident 2012 (No.2) BEST PRACTICES Be alert of changing geological conditions in the area of active mining. Loose unstable ribs shall be taken down or adequately supported. Extra precaution shall be taken to support the ribs when mining height increases.
