Coal Mine Fatal Accident 2012 (No.2) Operator: McCoy Elkhorn Coal Corp. Mine:Mine No. 23 Accident Date:June 25, 2012 Classification: Rib Roll District: Pikeville Mine Type: Underground Employment: 121 FATAL ACCIDENT 2012

Coal Mine Fatal Accident 2012 (No.2) OVERVIEW On Monday, June 25, 2012, a 33-year old outby foreman with 7 years of experience was fatally injured while installing a wooden timber next to a loose coal rib. He was positioned next to a loose coal pillar (rib), when a section dislodged and struck him resulting in fatal injuries. Mine management had increased the height of the mine entry to 8 feet and 6 inches by cutting additional mine roof. This action increased the rock thickness along the top of the coal ribs. The underlying coal seam had been mined, allowing additional pressure to be exerted on the standing coal pillar.
