Phase II At least 30 days prior to the opening date of your school, the following documents shall be provided: 1.Certificate of Insurance issued by an insurance company licensed to do business in Arizona; 2.Evidence of a public meeting, if required by A.R.S. §15-183(C)(5), at least 30 days before the charter operator opens a site for the charter school; 3.Copy of certificate of occupancy for each school site, issued by the municipality; 4.State fire marshal/fire department inspection report for each school site; 5.County health certificate for each site at which students will be taught, if required.
Phase III The Board has implemented a policy that requires a charter operator to have a Certificate of Occupancy and Fire Marshals report on file in our office for each site prior to receipt of any equalization payments.
