FSR 08 — confined spaces INTENT: To eliminate or minimise the risks of fatalities, injuries and events arising from work in confined spaces What are the supporting documents you should refer: Primary Standard PS Confined Spaces PS Permit to Work HSE Information HSE Information – Determining a Confined Space Related Forms E-T-8-0962 Confined Space Permit

Critical Control 1 A SPECIFIC RISK ASSESSMENT IS CONDUCTED FOR EACH CONFINED SPACE BY COMPETENT PERSONS Insert information on the location of the confined spaces register

Critical Control 2 WORKERS, STAND BY PERSON AND SUPERVISORS ARE APPROPRIATELY TRAINED, CERTIFIED AND COMPETENT Insert specific training by role required for confined space working

Critical Control 3 ENTRY CONTROLLED BY A PERMIT SYSTEM Permit issuers will need to be identified within the Health and Safety Plan. Each confined space entry requires a specific risk assessment – this will contain information that you can use in your review to verify critical controls Provide a photo here for your specific person(s).
