WFPHA Global cCharter

We acknowledge the elders and peoples of the Aboriginal people who are the traditional custodians/owners of the lands where we are today. We also acknowledge other First Nations peoples who are with us today.

Background WFPHA PET Public Health Functions frameworks (US and EU) Public health competency sets

A Global Charter for the Public’s Health (WFPHA)

Methods Five competency sets were mapped against the Global Charter framework CAPHIA, ASPHER, CEPH, CP FPHM, UK FPHM (Canadian set is in revision, therefore not included) Two of us independently mapped the competencies and then discussed discrepancies

CAPHIA ASPHER Aus CP FPHM UK FPHM (6 Domains) ASPHER (5 Domains) CEPH (8 Domains) Aus CP FPHM UK FPHM (10 Domains) Health Monitoring and Surveillance Disease Prevention and Control Health Protection Health Promotion Health Policy, Planning and Management Evidence-based Professional Population Health Practice (19 competencies) Methods in Public Health Population Health & its Material Environmental Determinants Health Policy, Economics, Organisational Theory & Management Health Promotion: Health Education, Health Protection & Disease Prevention Ethics (72 competencies) Evidence-based Approaches to Public Health Public Health & Health Care Systems Planning & Management to Promote Health Policy in Public Health Leadership Communication Inter-professional Practice Systems Thinking (22 competencies) General Professional Practice Cultural Competency Information, Research & Evaluation Policy Health Promotion & Disease Prevention Health Sector Advocacy, Development & Management (70 competencies) … public health intelligence to survey and assess … health & wellbeing … evidence of effectiveness of health or wellbeing improvement interventions, programmes and services … Policy and strategy development and implementation Strategic leadership and collaborative working for health Health promotion, determinants of health and health communication Health protection Health and public health care Academic public health Professional personal and ethical development Integration and application of competences for consultant practice (86 competencies)

Results Very hard to compare: Competency sets are differently written CAPHIA – within each Domain each Competency is graded and can be used from undergrad to postgrad curriculum design. Suggested content included. ASHPER – complex; content-prescriptive; each Domain is compiled in a slightly different way CEPH – levels for different levels of staff from front line to management; written as outcomes rather than outputs. Complex document, with expected content provided based on functions of public health Aus CP FPHM (does not admit non-medics) – very medical/practice oriented; several are about competent medical practice rather than public health UK FPHM (fellowship obtainable for appropriately trained non-medics) – complex, content prescriptive, directed at safe public health practice

Results: WFPHA coverage WFPHA Global Charter for Public Health CAPHIA (6 Domains) ASHPHER (5 Domains) CEPH (8 Domains) RACP PHM (6 Domains) UK FPHM (10 Domains) Governance: public health legislation; health and cross-sector policy; strategy; financing; organisation; assurance: transparency, accountability and audit 2 3 5 2 3 2 3 4 8 1 2 3 5 6 1 2 3 4 5 6 7 8 9 (2) Information: surveillance, monitoring and evaluation; monitoring of health determinants; research and evidence; risk and innovation; dissemination and uptake 1 3 4 5 All 1 3 4 8 2 3 4 6 7 8 9 10 (3) Protection: international health regulation and co-ordination; health impact assessment; communicable disease control; emergency preparedness; occupational health; environmental health; climate change and sustainability 2 3 4 1 2 3 4 5 6 1 2 3 6 7 9 10 (4) Prevention: primary prevention: vaccination; secondary prevention: screening; tertiary prevention: evidence-based, community-based, integrated, person-centered quality healthcare and rehabilitation; healthcare management and planning 4 1 2 3 8 2 3 5 6 1 2 3 5 6 7 9 10 (5) Promotion: inequalities; environmental determinants; social and economic determinants; resilience; behaviour and health literacy; life-course; healthy settings 3 4 1 2 4 1 2 3 4 6 8 1 2 3 4 1 2 5 7 9 10 (6) Advocacy: leadership and ethics; health equity; social-mobilization and solidarity; education of the public; people-centred approach; voluntary community sector engagement; communications; sustainable development 2 3 4 5 4 5 (7) Capacity: workforce development for public health, health workers and wider workforce; workforce planning: numbers, resources, infrastructure; standards, curriculum, accreditation; capabilities, teaching and training

Discussion We note that competencies are about what is expected to be taught, but not pedagogical approaches to delivering content Mapping competencies is difficult – cultural context and different backgrounds (for example, in part they reflect temporal, geographical, and political problems occurring when they were written) Identification of shortfalls/gaps in competency sets All except CAPHIA are used for accreditation of MPH and/or school and/or faculty accreditation WFPHA Global Charter provides a mechanism for being able to compare competency sets, and is another mechanism to check public health course content; could be useful in considering course (re)design
