SIM PCMH Initiative All SIM PCMH care managers: Required to take the SIM PCMH Initiative self-study module, in addition to the CCM course self-study modules. SIM PCMH Initiative Self-study module should be completed prior to the CCM in-person training.

High Intensity Care Model Specific Requirements HICM care managers: Required to take the HICM self-study modules in addition to the CCM course self-study modules Introduction to the High Intensity Care Model HICM case study Self-study module and case study will be completed after attending the CCM course. Program overview In-home assessment Use of geriatric syndromes Care plan development Interdisciplinary team

What to do with Self Study Modules Listen to all self-study modules prior to coming to the in person class on day 3 of the training* Allow about a half an hour to listen to each module After listening to the recorded webinar complete the post test for each module Bring the completed post tests to the in person class on Day 3 *HICM self-study modules will be completed after attending the CCM in-person training

In Person Training Day 3 and 4 Time: 8:30 am – Training begins with introductions 4:30 pm – Training ends *Continental breakfast and lunch will be provided *Please remember to bring your completed post-tests

Conclusion Successful completion of the MiCMRC CCM course includes: Attendance in the introductory webinar Completion of the self study modules *(including additional program specific self-study modules) Completion of the post tests Attendance at the 2 day in person training days

Nursing CE Credits Rule 2(l) A maximum of 10 continuing education hours may be earned for participation in a workshop dealing with patient care issues offered by a health care organization or professional organization that falls outside the accepted methods of approval that have been adopted by the board.   You will receive 10 Nursing CE credits for your attendance and completion as described in above statement

Social Work CE Credits This course is approved by the Michigan Social Work Continuing Education Collaborative. Course approval #: 021816-01 Number of Social Work CE Contact Hours approved: 16

Self Management Training Requirements Additionally, Care Managers may be required to complete a Self Management training program approved by the Michigan Care Management Resource Center To access a list of approved training programs visit our home page at micmrc.org
