Trauma Charting Process Reminders For ALL traumas (Alpha, Bravo and Limited) All traumas must be charted on the paper Trauma Flowsheet through patient discharge OR Upon receipt of admission orders, then you may transition to EPIC – if you transition to EPIC, in the narrative write: “Admission orders received, see EPIC” All original Trauma Paperwork is to be turned in to the A.A.

So far I’ve had requests for… Chart Audits… Now audited weekly for the prior week Over-all much better- especially with narratives, but I & O still the most common fall-out. So far I’ve had requests for… Lemon Bars Brownies Ranger Cookies Apple Coffee Cake If your shift can have an over-all average of OVER 90% I will bake something for you!

MISSING Trauma Charting Charts have been found in the following places: In the corner of the last room the Trauma patient was in On the gurney Under the gurney Under the gurney mattress On the bedside table At the trauma Doc’s computer On the counter of the trauma’s admission room At the nurse’s station On top of the shredder Behind the shredder Under the shredder In the patient belonging’s bag Give Trauma Charts to the A.A.

Anything in common with Missing Charts?? Nope, not really Main thing to remember to avoid missing charts… Give Trauma Charts to the A.A.

Keep belongings with patient or send home with family Remember….. There have been patient comments after they have been admitted, that their cut clothes were thrown away in the E.R. And now they are missing their iPhone, Wallet etc… Keep belongings with patient or send home with family

Techs are an important part of the Trauma Team Remember….. ER Techs are expected to respond to all traumas! Please help them fulfill this expectation The primary nurse can quickly ‘release’ the Tech if their help is not needed, but this is usually after CT scan. Techs are an important part of the Trauma Team
