Why are We Here? CHCF has a commitment to improve chronic disease care in California Creating financial incentives and addressing regulatory barriers Fostering leadership Improving availability of clinical data Helping to diffuse better ideas Be sure to cite that a number of the CHCF Leadership fellows are present.

Ideas Are Only the Starting Point Learning how to “do” and then “doing” Avoiding the same pitfalls and creating shared efficiencies Having some support both within and outside our practice environments

We hope that you’ve collected some better ideas, and that you’ll put them into practice.

These “better ideas” plugs are the “fuel” for the California Improvement Network.

California Improvement Network “Big Tent” for existing quality programs Encompasses variation of approaches Focus on how best to spread better ideas Early attempt to share across public and private sectors (silos) Currently, there are public and private sector initiatives, local ones and statewide ones, ones that come from purchasers, payers, providers, and patients, ones focusing upon specific illnesses or specific population groups or specific geographies. 

Goals of the Improvement Network Rapidly disseminate successes Avoid dead-end experiences Connect complementary efforts Reduce redundancies Identify (and start to fill) gaps
