Transforming local health services in Horsham and Mid Sussex

Why we need a Big Conversation Nationally health and care services are facing significant challenges Rising demand GPs and local hospitals struggling to cope People are waiting too long to see for their treatment People are living longer Growing and aging population More people living with multiple long term conditions This is all putting extra strain on services

Our financial challenge The funding available has been unable to keep up with the growing demand on services Services currently cost more money than there is available Gap between what we can afford and the amount services costs We cannot carry on spending more money than we have We can do anything, but we can’t do everything We have to be open and honest about the fact there may be difficult decisions around some services that are not cost effective in the future

How are we transforming services Working in partnership with County Council, community and voluntary sector, hospital trusts, community trust and mental health trust. Developing local plans to bring health and social care services closer together Working with other organisations across wider region to transform and improve services - Sussex and East Surrey Sustainability and Transformation Partnership (STP)

What we want to achieve Improve health and care services and make them more affordable and sustainable for the future Work together across organisations to address the challenges we all face Ensure no part of the system operates in isolation Reduce variation of services – no “postcode lottery” Make best use of available resources

How today will work… Discussions of approx. 30 minutes Time for you to have three conversations (or stay with one) Come together at the end for a brief feedback session on key points Your feedback will be recorded and used to help future plans for services
