MOC/BBS/JSA/Incident Investigation /SEMS/Permit to Work Training Management of Change (MP024) Why do we have MOCs? To effectively manage changes in procedures and process When to use MOCs? Any time there is an important change in how we perform our jobs, or what we use to perform them Who creates MOCs? The management, project manager, or client can initiate an MOC Who does the MOCs effect? MOCs effect different workers based on the nature of the change involved – MOCs may affect all persons company-wide or just a few folks on a specific job What does the MOC do? MOCs are used to mitigate any problems involved in making the change as well as informing relevant persons of the changes to take place

Daily!!! This promotes a proactive mentality that is required for a safety program to meet or exceed goals in accident prevention and or reduction. Who is responsible for BBSM? Everyone!!! Training of personnel? We are currently following the procedures PEC, Premier Safe Land and Safe Gulf. What is done with data we collect? We trend our monthly to allow us to make corrections in our process as needed.

Use JSA should happen every day on the job site, and if job site conditions change Who creates JSA? The team leader and workers collaborate on JSA before the start of each work day Who does the JSA effect? The JSA effects the entire work team on the site Do we ever start a job without a JSA? NEVER!!!!

A process for determining the root cause of an incident, near miss, or recordable event When do we use it? Incident Investigations are used whenever an incident occurs either offshore or onshore to a team member on the job site Who conducts the investigation? CAN management conducts the investigation with information provided by the crew at the site of the incident What does this effect? Incident investigations are used to determine the cause of the incident, to determine if work practices need to be changed, and to prevent future incidents of this nature occurring again

SEMS is a program used offshore to combine several different safety processes in order to more effectively mitigate overall safety hazards to the personnel & environment Who does SEMS effect? SEMS blankets several policies and procedures which directly affect everyone working on an offshore facility. Some of the processes included in SEMS includes: Environmental Management Plan, Job Safety & Environmental Hazard, Behavior Based Safety Program, Management of Change, and Safe Work Practices

Permits are used by the client as a written authorization that the location and work to be performed have been evaluated by a qualified client person Who creates permits? The client representative creates the permits before the team begins work each day on the job site What types of permits are there? There are several different types of permits, depending on work being done - hot, cold, confined space, lock-out/tag-out are common types What does the permit do? The permits gives a summary of the work to be completed, who will be on the job and how long it will last, what the safety concerns are with this job, and who has authorized the work to take place Do we ever begin work without a permit, or perform tasks that are not listed in our permit? NEVER!!!
