Performance Evaluation 2015 Performance Evaluation Cycle Offline Self Assessment (filling in of forms, discussions with Supervisors) Open – Available in the performance management system and on intranet > F & A > HR > Performance Evaluation Forms. Online Uploading of forms into the system 1st February 2016 – 10th March 2016 Supervisor 1 & 2 ratings and comments in the system Closing of system for reporting 10th March 2016 5pm

Step 3 (Staff & Supervisor(s) OFFLINE PROCESS   Step 1 (Staff) Step 2(staff) Step 3 (Staff & Supervisor(s) Self Assessment Supervisor discussions Based on objectives set in 2015 Staff sets up meeting to discuss: Contribution to other units Staff Emails form to Mgt of staff, budgets etc Supervisors Contribution, achievements, challenges etc Drafting of future objects Future objectives Proposed training needs Training needs Comments Step 4 (1st Supervisor) Step 5 (1st Supervisor) 1st Supervisor evaluation rating & comments After discussion and finalization of form with staff 1st Supervisor (1st supervisor may ask staff to make amendments after discussion) emails form to staff 1st supervisor fills in his assessment, rating, comments on the form and emails completed form back to staff

ONLINE PROCESS Step 6 (Staff) Step 8 ( 2nd Supervisor)   ONLINE PROCESS Step 6 (Staff) Step 8 ( 2nd Supervisor) Upload of completed form Rating and comments online Staff logs into system, uploads form and submits 2nd supervisor logs into system, views completed form online, rates the staff, comments and submits By submitting, the form is automatically available to 1st supervisor in the system and cannot be withdrawn after submission. Ensure the form is completely filled by you as staff and 1st Supervisor By submitting, the form goes directly to HR Step 9 ( HR ) Step 7 ( 1st Supervisor) Consolidation of forms, ratings, comments Rating and comments in online system HR collates the form, ratings, comments 1st supervisor logs into the system and forwards to evaluation review panel and views the completed form, rates, types in comments and submits to Step 10 ( Panel Review) either 2nd supervisor (if applicable) or to HR Evaluation Panel Moderation The panel moderates the ratings and gives final ratings
