HV COIIN… COLLABORATE. IMPROVE. innovate. “Somebody said once that you don’t need to predict the future when you can create it. The Home Visiting Collaborative Improvement and Innovation Network (HV CoIIN) is the first-ever national improvement collaborative in home visiting at a time when home visiting may or may not become national policy. Contributions of the HV CoIIN can help decide such an outcome. HV COIIN… COLLABORATE. IMPROVE. innovate. Learning Session 2 - Back to the drawing board: Revisited driver diagrams Developed PDSAs for ongoing or new tests of change HV CoIIN Motto: “All teach. All learn.” What have we learned?

Start small and work your way up! We don’t need to climb Mount Everest to achieve change! We can take small steps and still make progress!

Local cqi teams Support each other’s efforts! Keep the team small (ideally 3-5 staff) Include someone from each position Include participants, when possible Schedule regular meetings Identify clear roles and expectations Share the responsibility. The CQI rep. is expected to share information, not to do all the work! Ask other staff, community organizations and families (past & present participants) for honest feedback You need enthusiasm on the team, but input from the skeptics! Be transparent, and don’t be afraid to fail! You learn as much from your failures as you do from successes. Support each other’s efforts!

Data, DATA, DATA!

HRSA benchmark reporting – We made it! Highlights (>85%): screening for maternal depression discussion of safety topics interbirth intervals    Lowlights: screening for domestic violence and referrals for positive screens referrals for suspected developmental delays administering the HOME/HFPI at six months data collection for the monthly benchmark forms and on the Perceived Stress Scale

HRSA benchmark reports This team, our partners and each site will receive an aggregate report with all of the benchmark data. The report will include the constructs for the state, and the state high and low on each construct. Sites will also receive a report illustrating where they stand on each measure. So, where do we go from here?

Brainstorm of Meeting details Objectives: Vision statement & goals Benchmark data by unidentified sites Real examples (PDSAs), highlight successes Self-assessment of team members – before meeting? Use FE driver diagram and then dive into other topics Learn common challenges/ knowledge gaps that will inform the CQI handbook Share strategies around data sharing and processes Agenda: Spend a lot of time on: CQI show-and- tell, lessons learned Use self-assessment results and understanding individual assets Exercise on documenting change and how it happened What are the possibilities for technical assistance around CQI? Lunch according to a topic of interest; provide ice breaker questions Job-related: likes, dislikes, challenges, individual strengths; Allison Lamont has reflective questions
