HOMER PREVENTION PROJECT ORGANIZATIONAL CHART* Other MAPP Subcommittees Connecting Community Resources Workgroup Healthy Lifestyle Choices Workgroup Substance Abuse & Domestic Violence Workgroup MAPP of SKP Homer Prevention Project Advisory Workgroup Homer Prevention Project Leadership Team One representative from each agency represented on the HPP Leadership Team Includes additional stakeholders representing such agencies as law enforcement, schools, local businesses, medical and social service providers, etc. Responsible for advising the content of the Homer Prevention Project One representative from each of the following agencies: CICADA, Haven House, The Center, South Peninsula Hospital, MAPP Oversees grant component of project including budget, reporting, ensuring grant guidelines are met, etc. Provides oversight to Project Coordinator Responsible for major grant related decisions under the advice of the Homer Prevention Project Advisory Workgroup Subcommittees Specific to HPP Goals Project Coordinator Other Related Community Projects *This organizational chart is intended as a conceptual guide to demonstrate relationships and framework for the Homer Prevention Project SPF SIG Org Chart 03-06-12
