UTILIZATION OF COMPUTED TOMOGRAPHY IN YANGON GENARAL HOSPITAL DR UTILIZATION OF COMPUTED TOMOGRAPHY IN YANGON GENARAL HOSPITAL DR. AYE MOE THET MYINT DR. KHAING LAY MON UNIVERSITY OF PUBLIC HEALTH

Introduction CT is one of the most widely used imaging technologies in the tertiary hospitals because of its superior diagnostic ability and widespread accessibility. In YGH,CT utilization is increased because CT scan may allow earlier diagnosis.

Introduction(Cont.) Other factors may account for increasing rate of CT utilization are; avoidance of admission in the Emergency Department and patient’s or family requests. Findings from this research will give information for administrators and policy makers in order to improve guideline for utilization of CT scan which in turn reduces radiation hazard and unnecessary loss of hospital budget.

Objectives of study To assess the utilization of computed tomography in Radiology Department of Yangon General Hospital during October 2017 To describe the CT utilization for emergency patients, inpatients and outpatients in Radiology Department To find out the proportion of abnormal CT result among the patient who utilized CT

Methodology Study design Cross sectional descriptive study Study period and area August 2018 to December 2018 Radiology Department, YGH` Study population Records from Radiology Department of YGH

Data analysis Data analysis was done by Microsoft excel 2010 and SPSS version 16

FINDINGS

Distribution of utilization of different CT scan machines

Age distribution of study population

Sex distribution of study population

Utilization of CT scan according to holidays and office days

Utilization of CT scan according to wards/departments

Utilization of CT scan according to regions

Results of CT scan

Detection of diseases by CT scan according to age groups

Detection of diseases by CT scan according to sex

Detection of diseases by CT scan according to wards/departments

Detection of diseases by CT scan according to regions

Detection of diseases by CT scan according to days

Discussion There are five CT machines in YGH. Among them,128-slice machine(emergency department),16-slice machine(modular complex building) and Siemen or 16-slice machine(emergency department) are the most useful. Among 2570 patients,1772 patients were scanned by 128-slice machine.

Discussion(Cont.) Because 128 slice CT scanner can be adapted to provide three dimensional images for any patients ,young or old of any size, including those with cardiac or respiratory condition. For 21 to 70 years age group, utilization was nearly 80% of study population because these age groups may be accidents or injuries and may be tension or unhealthy behaviours, hypertension or stroke.

Discussion(Cont.) The utilization of CT scan in male population was two times greater than female because male go outside and work, so there may be accidents or injuries. The utilization of CT scan was most common in medical ward and emergency department. In medical ward, there are stroke patients and in emergency department, there are patients due to RTA or accidents.

Discussion(Cont.) The most utilized body parts were head and neck regions because there are patients due to RTA or accidents . During holidays, more normal results were seen than office days because in holidays junior medical officer decide whether CT scan need or not and there may be police cases for exact opinion.

Conclusion One out of the five CT scans had no abnormal findings. Hence the utilization of CT scan was optimal and effective. The percentage of no abnormal findings was higher in holidays(24%) than in office days and this finding was significantly different (p=0.006).

Recommendation There should be proper guideline for CT scan utilization to minimize unnecessary or overutilization, radiation hazard and hospital budget. Paper record of Radiology Department should be upgraded to electronic record and completeness of records should be checked thoroughly. Maintenance of CT machines should be promoted and non-functioning machine should be repaired urgently.
