In Preparation for 7th Grade: State immunization requirements: Tetanus/Tdap booster Meningitis vaccine 2nd Varicella vaccine or history of Chicken Pox disease If you are unsure about your child’s immunization status, contact your doctor’s office. Be sure to tell them your child is going into the 7th grade to ensure they are compliant for next year’s requirements. The immunization records can be submitted to the clinic via fax, e-mail, or in person beginning now! Please note: Do not send immunization records to the coaches with a physical OR send athletic physical forms to the nurses! Athletics: District Wide Physicals will be offered Saturday, May 13th 8:00-9:30 AM, at Highlander Stadium. Cost is $20.00 with checks payable to HP Sports Medicine.
