Registering on the system To obtain a log-in to the Registry, email customer.support@amplitude-clinical.com You will then receive an email with your temporary log-in credentials.

Patient Reported Outcome Scores The test system is for TEST patients ONLY. All patient data that is added to the test site will be purged during the upgrade to live. Patients will have the ability to self-register on the live system, by following the link on the ICRS website. The system will automatically email patients at pre-defined time periods, to request they log-on to the Patient Portal to complete the EQ5D and KOOS scores. The patient will be able to complete their scores on PCs, iPads and mobile devices at home or in clinic. If a patient does not respond to an email request to complete their scores, the system send an further two reminder emails. You can view any patients who do have not completed their Outcomes Scores on your Task List. From here you can send manual email reminders by clicking on the button. Alternatively, you can call the patient, according to your own process.
