BASELINE Record the outcomes that were observed at the beginning of your treatment. Clearly indicate the subjective and objective parameters of assessment. Include results of any investigations that were conducted.

BT AT Comparison Compare the objective and subjective observations at baseline and end point indicating improvement or deterioration.

Conclusions Explain your impressions on the effect of the treatment given. To what extent did it help to give relief to the patient. Are you sure that the effect, whether positive or negative was due to your treatment or do you think some other factors may have played a role? For instance, was the effect purely by chance?. Was the patient simultaneously on other treatments? If so, to what extent did your treatment contribute to the outcomes?.
