High Urgency Personal or family illness or emergency. Stuck due to flight cancellation. Urgent or emergent situation that makes it impossible for you to get to clinic or requires you to leave clinic urgently (OB continuity delivery, Jeopardy coverage). Medium Urgency Meeting, opportunity or other obligation that has arisen unexpectedly and meets the following criteria: 1. Requires your presence 2. Cannot be rescheduled Low Urgency Vacation or personal day. Trade for provider convenience. Meeting, opportunity or other obligation that was set 30 days or more in advance. Meeting, opportunity or other obligation that could be rescheduled, does not require your presence or a delegate could attend. Is the clinic or partial cancellation time frame (e.g. late start, early departure) half-filled or greater**? Cancellation approved* Any patients scheduled during the clinic or partial cancellation time frame (e.g. late start, early departure)? * Please review list of patients and communicate with clinic staff appropriate disposition (i.e. same day reschedule with another provider, reschedule to another clinic session with you, etc.).
