Supported Care Service Update from: Anne Borrows, Acting Assistant Director – Clinical Transformation

Case studies – Lily, George and Doris Thinking about how we can keep people safe and well in their homes for longer, and avoid admissions Comprehensive reablement when they are discharged

Two project group meetings have taken place since the last engagement panel The project Group proposed a community engagement and development component to the service model Community Action Norfolk and Norfolk Carers have been actively involved in developing the detail The model is being submitted to SNCCG Clinical Executive and Governing Body in February to agree the financial investment Implementation of the new service is expected three months after approval

Community Engagement and Development Connecting health and social care with the voluntary sector and wider community including monitoring and maintaining community assets.

Reablement Care Increased capacity within the Norfolk First support (NFS) service through both day and night.

Rapid Response Clinical Assessment and Review Clinical oversight from the integrated team including rapid response medical and social assessment of needs and review.
