HRA Completed 2.Review HRA & 3.Calculate BMI Child Healthy Weight Protocol Medical Provider Nurse Any Team Member LBHP/CC 4. < 5% (Underweight) 5. 5% - 84% (Normal) 6. > 85% (Overweight) Refer to PCP for Assessment & Evaluation Re-evaluate every 6 months or per doctor’s orders. Review doctor’s orders & continue to educate, monitor and coordinate until next evaluation. Health Promotion Well child exam, client education, general wellness. Re-evaluate annually Refer to obesity pathway. Re-evaluate every 6 months or per doctor’s orders.
