Healthcare Transition Project Introduction During 4 years of work of the Adolescent Forum, set up to improve LTHT services for young people, it was clear from staff feedback that transition was top of the agenda. They requested support to develop good transition of children and young people including all services in LTHT. A successful bid was submitted to the Burdett Trust for Nursing which enabled the trust to appoint a Lead Nurse/ Project manager and admin support. This will support the running of a 3 year Project that will develop a model of care for Healthcare Transition for Children and Young People (CYP) in LTHT. Objectives Method Develop a standard gold model of care for healthcare transition for all services in LTHT Produce a model of care that is flexible and adaptable to meet the needs of the individual young person and their families in each service To improve age appropriate care and services through integration and collaboration with network partners To be a leading Centre for Transition. We have set up a project control area in the Lead Nurse office, D floor Martin Wing. We will use tools & methods such as master schedules (that help us break down what needs to be done by who & when), process maps (to help visualise each process now & in the future) and standardisation (so that we agree the best way and all follow the same steps). With each of the objectives we have identified measures that will help us monitor as we change & confirm where we are improving.
