Age Your child has to be 4 years old on or before September 1, 2019. Only children whose birthdates are from Sept. 2, 2014 through Sept. 1, 2015 are eligible for participation in Pre-K.

2. Certified Birth Certificate (not a copy and not one from the hospital) Can be purchased from probate judge

3. Child’s Social Security Card If you do not have your child’s social security card, you will need to complete the following form today.

4. Proof of Address must have physical address----No PO Box Other bills taken as proof of address: current lease, property tax notice, homeowner’s insurance bill, mortgage statement, current PeachCare eligibility documents

If you are living with someone else, then--- Fill out the Proof of Residency form, then… You and the person you are living with get the form notarized. The person you are living with must bring or send a bill with a physical address as proof of their address. Make sure the bill and the form have the same physical address.
