PCMH Kids 2019 Expansion Opportunity Call for Applications Seeking 4 to 8 new primary care practice sites serving children (under 18 years old) of substantial payer and population diversity committed to transforming their practices to provide high-value patient-centered care. This call is targeting practices with electronic medical record systems and interested in the Patient Centered Medical Home (PCMH) model of care. Call for Applications Flyer

PCMH Kids Cohort 1 Sustainability PCMH Kids Cohort 1 will end their 3-year contract with the health plans on 12/31/18 PCMH Kids practices that meet OHIC PCMH RI definition will be eligible for commercial health plan sustainability payments effective 1/1/19 FAQs available, which highlight expectations Update on Sustainability Payments for PCMH Kids Cohort 1 PCMH FAQs 2018 With the assistance of actors, Chris Dorval-Donovan, and Selby Conrad, will continue where we left off at the March 28th, PCMH Kids BH Learning Collaborative, by expounding on motivational interviewing techniques and demonstrating the concepts through roll playing. Practices will have time to break out into groups to practice the techniques learned in this session.

PCMH Kids Cohort 1 Sustainability If you are part of a system of care (SOC) You are encouraged to reach out to your SOC leadership to discuss “next steps” in working with each of the health plans to ensure that the SOC successfully negotiates for PCMH Kids sustainability payment on your practice’s behalf. You will also want to have conversations with the SOC leadership to understand how your practice will realize the benefits of the sustainability payment so that your practice can continue to provide care coordination services and function as a patient centered medical home. With the assistance of actors, Chris Dorval-Donovan, and Selby Conrad, will continue where we left off at the March 28th, PCMH Kids BH Learning Collaborative, by expounding on motivational interviewing techniques and demonstrating the concepts through roll playing. Practices will have time to break out into groups to practice the techniques learned in this session.

PCMH Kids Cohort 1 Sustainability PCMH Kids leadership met with Managed Medicaid health plans (NHPRI and United) to discuss bridge funding that is needed for the PCMH Kids practices to successfully sustain the PCMH model between 1/1/19 and 6/30/19. It is anticipated that starting 7/1/19, the Managed Medicaid plans will be including PCMH Kids sustainability payments in the 7/1/19 budgets. With the assistance of actors, Chris Dorval-Donovan, and Selby Conrad, will continue where we left off at the March 28th, PCMH Kids BH Learning Collaborative, by expounding on motivational interviewing techniques and demonstrating the concepts through roll playing. Practices will have time to break out into groups to practice the techniques learned in this session.
