About me What is your name? How long have you lived in Southwark? Do you have a job or volunteer? If yes what do you do? What are your hobbies/ what do you like to do with your spare time? What are your likes and dislikes? When are you at your happiest? What health conditions do you have? (long term conditions, mental health, disabilities)

Interaction with services When did you start using health/ social care services? (GPs, hospitals, pharmacy services, district and community nursing) What happened? How did you feel at the time? What professionals did you see and speak to? (Including health, social care, voluntary organisations, charities) What do you think worked well? (good points) What didn’t work very well? (bad points) Do you take any medication? (who prescribes, what for, how much)

What could have helped more Did you have support from anywhere else? e.g. peer support groups, friends, family Did you feel you were able to make decisions about your care? Did you feel there were options available to you? Did you feel listened to and understood? Was there anything missing from the care you received?
