Session Objectives In this session, our panel of nationally recognized experts will: Provide real-world examples of effective strategies that individuals and their caregivers need to successfully care for advanced illness. Highlight current efforts to develop national guidelines and meaningful quality measures for advanced illness care. Identify future opportunities for more effective, evidence-based population health-level interventions.
