Secure Test Materials Consumable Test Booklet: Student responds directly in the test booklet. Standard answer documents are not provided. iLEAP Grades 3, 5, 6, and 7 Four different forms for each grade; one form per school English Language Arts (Writing only) and Mathematics LEAP Grades 4 and 8 Eight different forms for each grade; one form per school English Language Arts (Writing only) and Mathematics NOTE: Using the list provided at the January 2012 pretest workshop, the District Test Coordinator must verify the form number on school requests for additional consumable test booklets and when distributing district overage or additional consumable test booklets to schools.

Sample Consumable Test Booklet Form number is indicated at the top of the box in the upper right corner. A unique spine bar is placed on every document. The label box is not prepreinted with any student information. A bar-code label must be affixed to all consumable test booklets under the supervision of the School Test Coordinator.

Large-print booklets will be packaged with a consumable test booklet. Responses must be transferred to the consumable booklet packaged with the large-print booklet. o District or school overage cannot be used. Students using braille are not required to participate in field testing; therefore, braille booklets are not available for the field tests. Secure Test Materials Large Print and Braille

Other Test Materials Rough Draft Booklets (Secure) Contain only the brainstorming pages and the rough draft pages Generic version covers all grades and forms Must be returned to DRC with secure materials Writers Checklist (Secure) Grade 3 Grades 5, 6, and 7 Grades 4 and 8 Must be returned to DRC with secure materials Mathematics Reference Sheets (Secure) Provided only for grades 4, 6, 7, and 8 Must be returned to DRC with secure materials Rulers (Not secure) Provided only to grades 5 and 8; may be kept by schools Test Coordinators and Test Administration Manual (Not secure) Combined manual, which is different from other administrations

Mathematics Reference Sheets and Writers Checklists Identified as Field Test on the top of the materials Printed on white paper Must be returned to the District Test Coordinator Mathematics Reference Sheets and Writers Checklists must be returned with all secure materials to DRC. The operational iLEAP and Phase 2 of the LEAP tests begin April 12, and precautions must be taken to ensure the Field Test Mathematics Reference Sheets and Writers Checklists do not get mixed with operational Math Reference Sheets and Writers Checklists.

A bar-code label must be affixed to all consumable test booklets under the supervision of the School Test Coordinator. Bar-code labels are administration-specific (Field Test) and must be kept separate from all operational program labels. Student information on all consumable test booklets must be hand coded.

Unscorable Document Labels Use if a consumable test booklet was damaged soiled for another student marked with ink, highlighter, marker, or crayon Unscorable consumable test booklets must not be thrown away. Any responses must be transferred to a hand-coded consumable test booklet to be scored for the purpose of the field test. Documentation about the incident should be kept by the District Test Coordinator.

Packaging Materials for Return to DRC Collect materials from all schools and package them for return using the Field Test DRC return labels. NEW: Package scorable materials in the DRC Return Bags. Seal the bag with one of the provided plastic ties. Affix a Tangerine DRC return label to flap A. Package the nonscorable materials in a separate box. Affix WHITE DRC return labels to flap A of each box containing nonscorable materials. Affix UPS 2nd-day Air Return Service labels to flap B of each box. (Do not make copies of UPS labels.) NOTE: Scorable materials must be boxed separately from nonscorable materials, but are returned in the same shipment.

Summary of Exceptions The following systems and processes are not applicable to the Field Tests. TA System Online Surveys or Comment Sheets Makeup Testing Voids Provide LDOE with testing irregularity information. Scores will not be provided.
