Fig 1 Preoperative axial computed tomography scan showing an intact cortical envelope. This needs to be intact or easily re-established for this case to be successful. As depressed fragments are elevated, the cortical envelope allows them to be elevated and contained; if the cortical envelope is not intact, they will displace laterally or medially as they are elevated. Fig 2 Room setup showing the involved extremity in the usual arthroscopic position, except that the leg holder is placed higher on the leg to allow for better fluoroscopic images. The contralateral limb is abducted off the side of the operating room table to allow easy entrance of the fluoroscopy machine. The fluoroscopy machine comes in from the contralateral side, and the monitors for both fluoroscopy and arthroscopy are placed on the ipsilateral side for easy viewing by the surgeon. Fig 3 Anteroposterior and lateral radiographs with the guide pin placed approximately 1 to 2 cm distal to the depressed articular fragment. The pin is placed distal to allow elevation of the depressed fragment with a bone tamp under subchondral bone to prevent chondral perforation. This is performed after reaming over the guide pin with a rigid 10-mm reamer to make room for the bone tamp.
