Referral Process West Hampshire Community Diabtes Service (HCHC) Patient Referred by GP Single Point of Contact - Community Based Diabetes Specialist Service Nurse and or Consultant Triage of Referral Multidisciplinary Assessment Session for New Patients and / or onward referral to Type 1 or Type 2 Education sessions Secondary Care - not part of the routine service but for complications e.g. nephrology Follow-up sessions as required see pathways Discharge Patient Calls HCP calls DNS-led clinic Refer to GP/Practice nurse/District nurse Advice given nurse led or consultant supported sessions see pathways West Hampshire Community Diabtes Service (HCHC) 1
