What is my care record? New system which means Health Care Professionals can see patients health records while they are treating you. I.e. Hospital doctor can see info from GP. They can only do this if the patient has given permission and the records can not be copied, save or changed. It allows health and care professionals to make the best decisions about patients health.
What information will be seen? Patient name, address, NHS number and phone number. Medication. Results of tests. Copies of letters. What has happened in the past. Emergency department treatment. Appointment s. Health plans. Mental health issues. Social care lead co-ordinator and your care plan.
Who can see my record? WHAT IS THE BENEFIT FOR THE PATIENT? Only health and care professionals who are involved in the patients care can see the records. They need permission before they can see it. If there is an emergency they may be able to see your record without the patients permission. The system keeps a note of everyone who has seen your record. WHAT IS THE BENEFIT FOR THE PATIENT? Health and care professionals will work together better to give the patient the best care. Doctors can work quicker. Less paperwork. Patients will not be asked the same questions so often. Patients may need less tests.
Can I choose whether to give permission? Yes, patients can opt out of my care record. Opting out will mean that it will take doctors longer to work out the best way to help the patient. If a patient opts out, they can change their mind at any time. If a patient only wish’s to share part of their records, they will need to talk to their GP.

The Information Commissioner is investigating concerns that records held by 2,700 practices - one in three of those in England - can be accessed by hundreds of thousands of strangers. Privacy campaigners last night said the breach was “truly devastating” with millions of patients having no idea if their records had been compromised. Sharing out has not been switched on, this is not due to go ahead until July 2017, at this stage only information about my care record is being shared with patients. Patients records can only be made shareable with other health and care providers who are providing care to the patient and only after the patient has given consent.
